Certify that I am in generally good health and am capable of participating in the Chicago School of Canine Massage (CSCM) career training program, and that the information I have given in my application and during my interview is true and factual *
Unsatisfactory Conduct:
Acknowledge that CSCM reserves the right to dismiss any student from any class if the student demonstrates unsatisfactory conduct, is disruptive, is non-compliant or in any way causes concern for the instructor’s, students’ or animals' health and welfare, according to the policies of the school. *
Substance Abuse:
The Chicago School of Canine Massage (CSCM) believes that maintaining an environment that is safe, free from substance use/abuse, and conducive to learning is an important goal for the school and the community. Students are not permitted to possess, use, be under the influence of, or sell drugs, alcohol, or tobacco products on the school premises at any time, at any school sponsored activity, or when a student is traveling to and from school. Students violating this policy are placing themselves in jeopardy of representing CSCM in any capacity for reasons of personal safety for the individual and those around the individual.

We recognize our responsibility to address drug and alcohol problems in school and of our students. We believe that students and the school must work together to educate, encourage, and support students in an attempt to prevent their illegal use of drugs and alcohol. As part of our attempt to achieve the above, the schools will utilize the following policies: No student may provide any substance to any other student, by sale or any other means, which is represented to be a narcotic drug, hallucinogenic drug, amphetamine, barbiturate, marijuana, alcoholic beverage, stimulant, inhalant, legend drug, depressant, or intoxicant of any kind. This includes, but is not limited to any synthetic substance that may closely resemble any of the above substances or items. No student may possess, use, or be under the influence of any substance which that student has reason to believe is or which has been represented to be a narcotic drug, hallucinogenic drug, amphetamine, barbiturate, marijuana, alcoholic beverage, stimulant, inhalant, legend drug, depressant, or intoxicant of any kind. This includes, but is not limited to any synthetic that may closely resemble any of the above substances or items.

Students in violation of this policy are subject to appropriate disciplinary action, up to and including termination from CSCM programs without tuition refund. *
Informal Concerns, Complaints, or Grievances:
CSCM is dedicated to providing a safe, secure, thriving school environment that provides an effective means for students to voice informal concerns, complaints, or grievances. Informal complaints related to the classroom should be directed to an instructor. The instructor or school director will determine if the student’s complaint requires a verbal response, written response, or further action in order to resolve the specific complaint. All informal complaints or grievances are documented and kept on file indefinitely, for future reference if necessary. *
Dog Attendance Policy:
Acknowledge that if I bring a dog to class, I will abide by the conditions set forth in the Dog Attendance Policy and that CSCM reserves the right to seek the appropriate cure, including remedies available through the courts, for any monetary losses, damages or claims arising out of any damage or event caused by or resulting from the actions of my animal for the duration of the class or program. *
Missed or Make-up Hours:
In a certification program, the attending of all classes is required. Make-up work is the responsibility of each individual student. Missed hours MUST be made up. The course director will work with the student in scheduling the make up hours. All missed “hands on” hours will be completed through private instruction at $60/hour. *
Tuition & Fees:
Agree to pay CSCM the total stated tuition and fees. A non-refundable deposit is due at the time of registration and tuition in full is due two weeks prior to the start of class. Any required deposit is applied to the full class tuition cost. Students receive their course materials once tuition in full is received. No financial assistance or payment plan is available. Tuition balance is refundable if the student withdrawals at least two weeks prior to the start of class. If the student withdraws less than two weeks prior to the start of class or after the program has begun, tuition is non-refundable, unless the withdrawal is due to extenuating circumstances. Extenuating circumstances include illness, injury, bereavement, military obligations, or other if approved by the Chicago School of Canine Massage Board of Directors. Any extenuating circumstance must be submitted in writing to Chicago School of Canine Massage and approved by the Board of Directors. *
Program Cancellation:
Courses are subject to cancellation by Chicago School of Canine Massage due to limited enrollment one week prior to the program start date. If a program is cancelled by Chicago School of Canine Massage, students are subject to a full tuition refund or can enroll in a future program. *
Act of Nature, Natural Disasters, Act of War, Contagious Pathological Outbreaks:
Chicago School of Canine Massage reserves the right to cancel a program up to 24 hours before the program start date due to act of nature, natural disasters, act of war, contagious pathological outbreaks (both human and canine), or other if deemed appropriate by the Chicago School of Canine Massage Board of Directors. If a program is cancelled, students are subject to a full tuition refund or can enroll in a future program. *
Out of Town Travelers:
If you are traveling to a class location from out of town, you will need to make arrangements for accommodations. Your program costs do not include transportation, meals or lodging. *
State Laws & Regulations:
We strongly encourage you to read the individual state law pertaining to the state in which you intend to practice. Please note that state laws change constantly and are often left up to interpretation. CSCM, its owners and employees will not be held accountable for any information gained from any source.  *
Placement Assistance:
The Chicago School of Canine Massage does not have a placement assistance program upon completion of programs. However, CSCM's follow up includes help and direction at being successful in the canine massage industry. CSCM will provide students with letters of completion, certifications, and letters of recommendation. *
Losses Incurred:
Release and hold harmless CSCM for any losses incurred or charges stemming from travel arrangements, lodging, airfare, etc. caused by class cancellations due to bad weather or for any other reasons deemed appropriate by the school. *
Transferability of Certification:
The Chicago School of Canine Massage programs, including: Level 1 Canine Massage Foundations, Level 2 Canine Massage, 10 Month Advanced Canine Massage, Mentorship Advanced Canine Massage, and Post-Surgical Massage and Edema Management for Veterinary Professionals, have all been recognized by the National Board of Certification for Animal Acupressure and Massage (NBCAAM).

National Board of Certification for Animal Acupressure and Massage (NBCAAM) endeavors to provide examinations that reflect and uphold the highest professional standards of practice for animal acupressure and massage. To that end, the examinations are available to people who have completed specialized animal training programs in either or both disciplines. Upon successful completion of the examination in the chosen discipline, the practitioner will be deemed a knowledgeable and professional animal acupressure or massage practitioner and will be recognized as such by noted professionals practicing in that particular field.

If a student is interested in transferability of certification to other institutions of higher education, please consult with said institution for transfer approval. *
General Concerns, Grievances, or Disputes:
Students can submit any complaints, grievances, or disputes to the Illinois Board of Higher Education.  COMPLAINTS AGAINST THIS SCHOOL MAY BE REGISTERED WITH THE BOARD OF HIGHER EDUCATION.

Photography or Audio/Video Recording:
I authorize CSCM to take and utilize at their discretion photographs and or audio/video recordings or any material submissions taken during any classes, activities and/or events. These photographs and or audio/video recordings may be utilized for any CSCM promotional purpose. Students are allowed, with permission, to photograph and videotape for personal educational purposes only. Any video and photography usage other than permitted will be prosecuted to the full extent of the law. *
Completion of Materials for Certification:
Students are required to complete all program coursework within six months of class start date to receive certification. An established due date will be provided on your class syllabus. All materials submitted after due date will be classified as incomplete. A student who receive an incomplete will be required to retake a CSCM program for certification status. *
Future Claims, Including Third Party Claims:
Release and hold harmless Chicago School of Canine Massage and any hired or unpaid agents, contractors, vendors or lessors, acting as a representation of the aforementioned companies and person (collectively referred to as “CSCM Representatives”), from any and all future claims, including third party claims, arising from liability due to accident, damage or injury to self, horse, dog, or others, and/or loss of or damage to personal property during, or resulting from, any Chicago School of Canine massage classes, programs or events. Release and hold harmless Chicago School of Canine Massage and CSCM Representatives from any injury etc. to my person or property resulting from the inherent risks of the classes or programs, events, or the activities resulting from any action or inaction by CSCM or CSCM Representatives. This waiver and release is effective even if the injury or damage to person or property is caused by or contributed to by actions or failure to act of CSCM or CSCM Representatives. And which actions or inactions constitute ordinary negligence of a violation of any applicable law pertaining to canine activity liabilities. Neither Participant nor Participant’s representatives shall make any claim against, maintain an action against, or recover from CSCM or CSCM Representatives or its independent contractors, instructors, directors, officers, members, employees, agents, volunteers, lessors, representative, designated officials, or others acting on their behalf for injury, loss or damage of the participant, participants invitees, participants canine or other animal, to the participant’s personal property (regardless of ordinary negligence by CSCM or CSCM Representatives or regardless of an alleged violation of an applicable feline or canine activity liability law.) *
Acknowledgement of Risks:
I hereby acknowledge that there are numerous inherent risks associated with being on the property, including but not limited to risks associated with canine activities, activities involving other animals, activities around property and equipment and other hazards associated with entering onto or being on the Property, whether preparing for, entering, attending, participating or leaving any CSCM classes or programs, or being on the Property. The inherent risks include those dangers and conditions which are an integral part of canine activities or activities involving other animals, other activities, or being on the Property, including but not limited to: the propensity of a canine to behave in ways that may result in injury, harm to persons on or around them; the unpredictability of the canine’s reaction to such things as sounds, sudden movements and unfamiliar objects, persons or other animals; certain hazards such as surface or subsurface conditions; collisions with other animals or objects; the potential of participant or others to act in a negligent manner that may contribute to injury to the participant, participant’s canine or participant’s other property (such as participant’s vehicle or personal possessions) or others such as failing to maintain control over the canine or not acting within his or her ability; the breakage or failure of tack or equipment or property; the potential that an feline or canine may cause injury or harm to the participant or other persons or animals in the vicinity. Participant is not relying on CSCM to list within this document all possible inherent risks or all risks of participating in any of the activities on the property or at any other location. *
Acceptance of Terms and Conditions:
This document is intended to be as broad and inclusive as any applicable law permits. If any clause conflicts with applicable law, only that clause will be void but the remainder shall stay in full force and effect. Execution of this form denotes my acceptance of the terms and conditions described above and within the “Release of Liability and Risk Assumption Waiver.” I hereby release and hold harmless the Chicago School of Canine Massage (CSCM), Canine Massage Chicago, The Anti-Cruelty Society, ForYour K9, Integrative Pet Care, Animal Care League, Chicago Animal Care and Control, Safe Humane Chicago, and any and all of CSCM’s instructors, partners, guest speakers, volunteers, demonstrators from any liability due to accident or damage to self, horse, dog, cat or other animal; and/or loss of or damage to personal property during any CSCM class, session, lesson, event or other form of demonstration or learning. My signature below and submission of this form denotes my acceptance of the terms and conditions described above. I HAVE READ THIS ASSUMPTION OF RISK, WAIVER AND RELEASE OF LIABILITY, AND I UNDERSTAND THAT IT IS A RELEASE OF CLAIMS AND THAT I AM ASSUMING RISKS INHERENT TO MY PARTICIPATION, AND I AGREE TO BE FULLY BOUND BY ITS TERMS. *
